The Modernisation Initiative End of Life Care Programme developed the AMBER care bundle with a clinical team in Guy's and St Thomas' NHS Foundation Trust. The Modernisation Initiative is funded by Guy's and St Thomas' , King's College Hospital Charity and South London and the Maudsley charities.
The Transform Programme aims to improve the quality of end of life care within acute hospitals across England, enabling more people to be supported to live and die well in their preferred place.
The programme focuses on both the quality of care provided by acute hospitals, as well as the important role acute hospitals have, as one of many organisations that may provide care for people who are approaching end of life. End of life care includes care for people in their last years, months and days of life as well as care after death.
An independent not-for profit-organisation based in Cambridge, Massachusetts, IHI focuses on motivating and building the will for change; identifying and testing new models of care in partnership with both patients and health care professionals; and ensuring the broadest possible adoption of best practices and effective innovations. The IHI developed the concept of "reliable design" and "care bundles" used by the AMBER care bundle design team.
The NHS Change Model has been created to support the NHS to adopt a shared approach to leading change and transformation. The NHS Change Model brings together collective improvement knowledge and experience from across the NHS. It has been developed with hundreds of our senior leaders, clinicians, commissioners, providers and improvement activists who want to get involved in building the energy for change across the NHS by adopting a systematic and sustainable approach to improving quality of care.
